Bridging the gap with advanced technologies: Integrating AI and multiomics ctDNA analysis in multicancer screening 

Dr. Le Son Tran (Gene Solutions, Vietnam) effectively transitioned the discussion from clinical challenges to technological innovation. He described MultiCancer Screening as a potential “game changer,” particularly for cancers that currently lack effective screening options. Howeverhe also emphasized the central scientific constraint: ctDNA signals in early-stage tumors are typically low, variable, and cancertype dependent. This demands exceptionally high sensitivity without sacrificing specificity. He suggested that multi-omics analysis combined with AI-driven machine learning could be a promising approach to meet this requirement. 

In his view, the path forward depends on effectively applying such innovative technologies while ensuring that screening remains practical, cost-effective, accessible, and sufficiently simple to integrate seamlessly into routine clinical workflows. To address these challenges, he presented  SPOTMAS, an advanced assay designed to screen 10 cancer types through a single blood draw. This platform leverages multi-omics analysis of genetic, epigenetic, and fragmentomic signatures of cell-free DNA, combined with AI-driven machine learning, to optimize detection performance and accurately predict the tissue of origin. 

These advancements are firmly grounded in robust analytical and clinical evidence. Dr Tran highlighted results from the prospective KDETEK study which enrolled over 9,000 asymptomatic participants. The study demonstrated 78% sensitivity and 99% specificity, with strong performance in earlystage asymptomatic cancers. His vision was summarized in the following statement: “We believe that multiomics analysis of cfDNA combined with machine learning and simple workflow could be considered as the next generation of MCED assays.” 

Clinically, Dr. Tran advocated positioning MCED as a complementary tool to current guideline-recommended screening, while emphasizing the critical need for randomized controlled trials to confirm long-term patient outcome benefits.
